Personal Action Toward Health (PATH) is a six-week workshop, conducted by trained leaders, designed to help people with long-term health conditions improve overall well-being and manage symptoms. PATH is an evidence-based program developed by Stanford University and also known as the Stanford Chronic Disease Self-Management Program. Anyone who has a chronic condition or lives with and/or cares for someone with a chronic condition is encouraged to participate in a workshop. Workshop leaders can be health professionals or lay leaders – all of whom have completed a leader training course.
